Fix my gun Canuck Operator bolt head failure

Buddy brought out his Canuck Operator tonight and we ran it like we usually do. We were shooting a mix of miscellaneous ammo: target loads, 3” steel birdshot, and 2 3/4" slugs.

On the last string, we had a failure to extract. Upon inspection, the shell was expanded and stuck in the chamber, the primer was sticking out of the brass, and the bolt head basically exploded.

Never seen this happen before.

The fact that the primer was sticking out kinda makes me think this might have been a double charge from faulty ammo. The only other logical thing could be a 3.5" in a 3" chamber maybe.

If the lugs failed then nothing is holding the shell to the bolt face and the primer can be pushed out.
