S&W 1854 357 jamming

So far I’ve put 250 rounds through her.
This has happened a few times.
Example here in the video.
Today at the range I shot 100 rounds and this happened about 4 times.
Reasons?

Get good, then get fast. Slow down, and fully cycle the action.
