Canuck Operator bolt head failure

Buddy brought out his Canuck Operator tonight and we ran it like we usually do. We were shooting a mix of miscellaneous ammo: target loads, 3” steel birdshot, and 2 3/4" slugs.

On the last string, we had a failure to extract. Upon inspection, the shell was expanded and stuck in the chamber, the primer was sticking out of the brass, and the bolt head basically exploded.

Never seen this happen before.

I see there was a competition here, Turkish shot gun vs Savage mk II mag. Does the turknelli catastrophically fail before the mark 2 mag falls apart? It would seem yes
